Job Summary
Our client is seeking an experienced MDS Coordinator to join their team. This role involves analyzing nursing care needs, implementing and coordinating MDS/MMDS programs in compliance with State and Federal regulations, and ensuring accurate and up-to-date care plans for residents.
Key Responsibilities
- Analyze nursing care needs of residents and write appropriate care plans enabling staff to provide personalized nursing care.
- Implement and coordinate MDS/MMDS program to ensure compliance with State and Federal regulations and review and revise procedures annually.
- Set up monthly Care Plan schedule.
- Participate in nursing committees and meetings as needed and update knowledge and skills through seminars, workshops, and continuing education activities.
- Work cooperatively and tactfully with residents, staff, family, Administration, DON, and consultants.
- Ensure initial and ongoing care plans are up to date and accurate.
- Transmit MDS information to the State within the compliant time frame.
- Maintain current knowledge of resident status to be reflected in MDS.
- Facilitate medical assistance review, assuring appropriate documentation for residents’ level of care and reimbursement with assistance from the DON.
- Complete ADL coding audits relating to MDS completion and compliance. Forward audits to Administrator and Director of Nursing for review and action. The DON will report audit results to the Q.AP.I. Committee quarterly.
- Participate as a nursing team member to implement care plans generated by CAA protocol.
- Pull, review, and ensure accuracy of the 672 and 802 monthly, communicate any discrepancies first to the DON, and provide accurate reports to Administrator and Executive Director monthly.
