Job Description
As an integral member of the Project SHINE clinical care management team, the Registered Nurse (RN) Care Manager will play a crucial role in supporting individuals living with HIV/AIDS and those at risk for HIV transmission. This position offers the opportunity to work with diverse, high-need populations, helping them overcome barriers to self-care while ensuring comprehensive, patient-centered care. The RN will be responsible for a variety of tasks, including patient education, medication administration, case management, and collaboration with multidisciplinary teams to develop individualized care plans.
Responsibilities/ Job Duties:
- Assess and perform nursing intakes, obtaining patient history and conducting appropriate tests.
- Provide clinical follow-up, including addressing care gaps, vaccinations, and screenings.
- Work with PCP/ID Specialists and the HIV team to develop individualized care plans and adherence strategies.
- Lead and participate in multidisciplinary care teams, promoting teamwork and communication.
- Educate patients on chronic disease management, HIV care, STI screening, and safe sex practices.
- Administer medications, treatments, and manage medication refills as directed.
- Document patient histories, assessments, plans, and treatments accurately in EPIC.
- Implement Domestic Violence and Abuse Screening policies and ensure infection control standards.
- Participate in quality improvement initiatives and contribute to grant writing and reporting activities.
- Manage STD symptom screening, referrals, and follow-up care for PrEP and PEP patients.
- Support medical providers in interpreting STI lab results and linking HIV-positive patients to care.
- Assist with case management for patients in the PrEP and PEP programs, ensuring adequate follow-up care.
Minimum Education and Experience Qualification Requirements:
- MA RN License
- BLS Certification
- 2-3 years of RN experience, preferably in a community health setting
- Experience working with diverse patient populations or FQHC experience
Nice to Have Skills:
- HIV or STI experience
- Bilingual (English/Spanish)
- Experience with EPIC
